Applicants are advised to use only Google or Yahoo email accounts in securing an appointment to avoid any technical incompatibilities. This appointment and scheduling system allocates slots on a first-come, first-served basis.Applicants should accept the responsibility for supplying, checking, and verifying the accuracy and correctness of the information they provide on DFA system in connection with their application. Incorrect or inaccurate information supplied may result in forfeiture of passport application.
For Sites utilizing the ePayment System, all fees are non-refundable. Fees shall be forfeited for applicants who fail to show up on their confirmed appointment, applicants who cancel their appointment, applicants whose application was rejected due to inconsistency and/or incorrect information, and applicants who present discrepant and/or spurious documents.
To book an appointment, visit the official website, https://www.passport.gov.ph/appointment. Click you have agreed to the terms and conditions and choose start individual appointment of start group appointment. Then you will be redirected to fill-up the sample form below:
Select Region, Asia Pacific if you are in Philippines. Choose Site, and once you choose the location where you want to apply for a passport, the slots on the right side will be automatically filled-up as per your chosen location. At the bottom of the page, click Please check if you agreethen proceed to the "Next" page.
Here you will see the avilable slots at Robinson's Las Pinasw where the earliest availabe date is March 20, 2023. Choose the date you wanted and the timing will appear on the right side of the page. Click I'm not a robot then proceed to the next page.
Then you will be prompet to the page where you need to enter your personal details. Good luck to all!
