2. Photocopy ng birth certificate ng bata

3. 2 pictures ng bata isang 3r na fullbody at isang 3r na close up

4. Barangay certification or Valid ID (SSS ID, VOTER’S ID, PASSPORT, POSTAL ID) ng Magulang or Guardian

5. HUWAG PO MUNANG ISAMA ANG BATA BUKAS SA REGISTRATION
